Service Overview
The Forensic Outreach Services Program provides specialized mental health services to individuals found “Unfit to Stand Trial” or “Not Criminally Responsible” and living in the community under a disposition order of the Ontario Review Board throughout the Northeast Region.
Services Provided
Treatment and rehabilitation is individualized, focused on recovery and community reintegration, and provided by a multidisciplinary team.
Outreach Services
Transitional Case Managers
Transitional Case Management is provided to assist patients with discharge planning and provides community support once a patient is considered ready for reintegration into the community.
Their contact with the patient begins in hospital, once discharge has been identified as a viable option. They will make referrals to community mental health & housing agencies such as Homes For Special Care and ACT teams. The goal is to have patients returned to their home community with sufficient supports in place to ensure successful reintegration. Once the patient is placed in the community, the TCM will continue to provide support in conjunction with the Forensic Outreach Clinician until their services are no longer required by the patient.
Nursing
Forensic Outreach Nurses provide ongoing psychiatric and psychosocial support to all individuals on a disposition order from the Ontario Review Board and living in the community. They remain involved until the patient receives an absolute discharge and are responsible for risk management and ensuring their patients comply with the terms of their disposition order. They work closely with all community agencies involved in their patient’s care and set up rehabilitation plans to address each patient’s individual needs. They are also responsible for giving input into the hospital report to the Ontario Review Board for annual and restriction of liberty hearings. Due to their role in the supervision and monitoring of these patients, their testimony is often required at these hearings. Forensic Outreach Clinicians are on–call 24/7 for emergencies after hours.
Psychiatry
Psychiatric follow-up and treatment is provided, as well as consultation to another community physician who is providing direct clinical care.
The Outreach psychiatrists are directly involved in the writing of the hospital report to the ORB and provide testimony during the annual hearings.
The Forensic Outreach Service has a close working relationship with Eagle Lodge, Forensics Inpatient Unit located at the North Bay Regional Health Centre.
